A very difficult lure to find especially new in the package. This is either the Creek Chub Cheekie C-112 or C-100 and it was made by The Creek Chub Bait Company in response to compete with the popular Lake Erie's own Erie Deerie. I know of no others for sale and I haven't seen anymore except the couple I have for sale in nearly 20 years.
